Life/AD&D insurance is with Reliance Standard Life Insurance Co. Eligible employees receive a life insurance benefit of two times their salary, rounded up to the next thousand. AD&D (Accidental Death & Dismemberment) has a schedule of benefits for loss of life or limbs due to an accident, up to the life benefit. Beneficiary change forms are available on request from the Manager of Benefits.
